1. Eligibility and Enrollment
Our Smile Dental Plan is open to individuals and families. To enroll, you need to be at least 18 years old, or a
parent/guardian can sign up for minors. Joining is easy! Simply inform one of our front desk staff that you are
interested, from there our team will assist you.
2. About Our Plan
The Smile Dental Plan is not an insurance policy; it is a renewable annual membership program (1 year)
designed to provide you access to our special membership-only pricing for dental service.
With the Smile Dental Plan, you enjoy comprehensive coverage that prioritizes your needs. Our special
membership fees are designed to fit your budget and provide valued savings on comprehensive dental
treatments.

4. Payment Terms
We provide flexible payment options, accepting both credit and debit cards. However, please note that we do
not offer payment plans. A yearly activation fee of $500 is required for plan activation and renewal. All fees are
subject to change based on market conditions.
Military personnel or veterans opting for the full Smile Dental Plan benefits will receive a 50% reduction in the
enrollment fee.
5. Cancellation Policy
If your circumstances change, you can cancel your membership by contacting us via phone, email, or inperson within 30 days of activation. Please note that there will be NO Refund after exams or treatments have
been used.
6. Communication
We believe in staying connected with our members. You’ll receive updates about your plan and exclusive
offers via email, sms, and our app. Your feedback is important to us, so feel free to reach out with any
suggestions or concerns.
7. Modifications to Terms
We may update these terms as needed. If there are significant changes, we will notify you via email, SMS, or a
phone call, so you’re always in the loop.

patients will be notified in advance via emails or phone calls. Current members will receive any newly added
benefits when plans are updated, at no additional cost.
